Please note that the form field has a limit of 255 characters. If it is over 255 characters, the form will not be saved.
You can start with a blank PDF, add the required text and labels in the PDF, and then add form fields to complete the form.
In Acrobat, choose Tools > Prepare Form.
Choose Create New, and click Start.
Remember to Save the PDF: In the upper-left corner of Acrobat window, click the Save icon.
Note: If you don't see the Create New option, update your Acrobat. To automatically update from Acrobat, choose Help > Check for updates, then follow the instructions in the Updater window to complete the update process.
3. Click Add Text in the toolbar, and add text or any other information to your form.
4. To add a logo or image to the form, choose the Add Image tool in the toolbar, select the image, click OK, and then place it at the desired location. Adjust the size or location of the image as appropriate.
Below is an example form with labels created using the Add Text and the Add Image tools.
5. To change existing text or images, click Edit in the toolbar. All the text and image fields are enabled for editing.
6. Add fields using the form field tools in the toolbar. Delete, resize, or arrange the fields as needed. To add a field:
In the toolbar, click the tool corresponding to the field type, or right-click anywhere in the form and then choose the tool.
Click at the desired location in the form to place the selected field.
Set the field properties. The field properties vary based on the type of the form field.
7. While you’re adding fields, click Preview to see how they look. Click Edit to come back to the editing mode.

Once you complete adding form fields, save the form.

PDF Form Fields Properties
Please Note: For IIQ to correctly import the PDF, all form fields must be set up in a proper format.
For instance, when importing into IIQ a form that will contain a signature, the imported form field should be a signature field in the PDF form.
Here’s a quick overview of how to add a signature fill block in Adobe Acrobat.
Once you have your file in a PDF format, open it in a PDF editor like Adobe Acrobat. Then, follow these steps to add a signature block to the form:
Click the Tools tab at the top of the window.
Choose Prepare Form.
Select Insert a Signature Field.
This will let you draw a fillable signature box anywhere on the form. Repeat the process for as many signature blocks as you need. Press Save, and you’re ready to sign.
